TERMS & CONDITIONS

Welcome to Revive Health & Fitness, trading as Revive Health & Fitness Limited. We aim to provide facilities, which meet and exceed your expectations. If at any time you feel we have failed to meet these standards, please do not hesitate to contact a member of our team or complete a customer comment feedback form. Every effort will be made to ensure your visit is a pleasant one.

 

Terms and Conditions
  1. This membership is offered for a minimum of 3, 6, or 12 months as chosen by the member, this is your contract obligation period. If the direct debit is cancelled within this obligation period you will be required to make payment for the remainder of the term. To cancel your membership you must give one month’s written notice after the last obligated month of membership. If you do not send us written notice to cancel your membership will continue on a monthly rolling basis until this is received.

  2. Please note that it is your responsibility to ensure the cancellation request has been received and actioned. It is not valid until you receive an email or letter acknowledging and confirming acceptance of the cancellation request.

  3. The Direct Debit contract will commence on the date stated on the front of this form under the Date of the first Direct Debit.

  4. During your obligation period, only in exceptional circumstances, such as injury, pregnancy, redundancy, or moving away, can you terminate your membership. You must provide proof by written document for us to process the termination. We reserve the right to decline your application if we feel your proof of document is not accurate.

  5. If you are pregnant or injured, you can freeze your membership free of charge but must provide a letter of proof from your doctor.

  6. During your initial obligation period you can transfer your Direct Debit membership to a friend or a relative who can then take over the remainder of your existing membership.

  7. All members are required to show either their membership access fob or device with a membership QR code to access the facilities. A photograph will be required on joining for identification purposes. Members may be refused entry into the club without a valid membership access fob or barcode.

  8. There is a small charge of £5 to replace lost or stolen access fobs.

  9. To attend classes you must book in to be guaranteed a space. You must comply with our time slots and if you attend at a later time you may be refused entry.

  10. Please make every effort to advise the club if not attending a session. This will allow other customers to utilise the space.

  11. We will write to inform you a minimum of two weeks before the introduction of any price increase. Please inform us if your personal details change e.g. address, telephone number, and email.

  12. You must use all equipment and facilities in an appropriate manner and give safe regard to your own health and safety and to that if others.

  13. We reserve the right to withdraw all or any part of our facilities for periods where we require them for exhibitions or in connection with the repair, alteration, or maintenance work. We also reserve the right to withdraw or change any publicised class list due to exceptional circumstances e.g. instructor sickness.

  14. We accept no liability for lost or stolen goods whilst on our premises unless as a direct result of our negligence. Property is stored in a locker at your own risk. Cars parked in the vicinity and all contents in them are your responsibility and we will not accept any liability for loss or damage to them.

  15. It is your responsibility to ensure that you are fit to use the facilities and you should inform us in writing, or a member of the fitness team if your health or any medical conditions/medications change. It is the responsibility of the member to ensure they use the facilities provided under this agreement. Revive Health & Fitness Limited takes no responsibility for non-attendance and as such refunds will not be issued due to non-attendance.

  16. Members must comply with conditions of use throughout the facility relevant to each activity.

  17. Revive Health & Fitness Limited reserves the right to adjust or amend the terms and conditions of use.

  18. We reserve the right to cancel a membership if these conditions are not adhered to.  

 

The Direct Debit Guarantee
  • This guarantee is offered by all banks and building societies that accept instructions to pay Direct Debits.

  • If there are any changes to the amount, data or frequency of your Direct Debit, Revive Health & Fitness Limited will notify you 7 working days in advance of your account being debited or as otherwise agreed.

  • If you request Revive Health & Fitness Limited to collect a payment, confirmation of the amount and date will be given to you at the time of request.

  • If an error is made in the payment of your Direct Debit by Revive Health & Fitness Limited or your bank or building society, you are entitled to a full and immediate refund of the amount paid from your bank or building society.

  • If you receive a refund you are not entitled to, you must pay it back when Revive Health & Fitness Limited ask you to.

  • You can cancel a Direct Debit at any time by simply contacting your bank or building society. Written confirmation may be required. Please also notify us.
